Reporting to the Director Clinical Services, the Hospice Nurse Navigator is responsible for the coordination of care for all patients being admitted to Citrus Valley Hospice. Responsible for the assessment and evaluation of patients for transition to the home settings. Promotes the Citrus Valley Hospice services to referral services and the community at large.

Bachelors degree preferred. Master`s degree/Nurse Practitioner preferred.

Minimum of 1 year experience in acute care and at least 1 year of prior Home Health experience required. Must demonstrate effective communication skills, expert leadership capabilities, be detailed oriented and capable of multi-tasking and responding rapidly to changing clinical situations. Highly motivated and self-directed, will demonstrate ability to function independently and exhibit high levels of independent problem solving skills and decision making capabilities. Excellent written and verbal communication skills required. Excellent customer service skills required.
